As part of an Ohio Department of Education reporting requirement, as well as that of the Every Student Succeeds Act (ESSA), the South-Western City School District is required to identify those students whose parent/guardian is an active member of the Armed Forces or National Guard.
If you are currently an active member of the military, please provide the following information and select the appropriate option using the form to the right.
NOTE: This form only needs to be completed if you are a SWCSD parent/guardian who is also currently an active member of the military. If more than one parent/guardian are an active member of the military please fill out the form for each parent/guardian.
