Immunization Clinics

NEW IMMUNIZATION REQUIREMENTS: By State law, your child must have a minimum number of REQUIRED doses of vaccine to attend school. Parents MUST provide records, within two weeks of enrolling, that indicate the shots are complete or are in process.

Beginning with the 2016-2017 school year, a new immunization requirement has been added. All students entering seventh grade are required to be vaccinated with one (1) doses of meningococcal vaccine prior to the start of the school year. All students entering the 12th grade are required to be vaccinated with two (2) doses meningococcal vaccine prior to the start of the school year. Parents must provide proof of the meningococcal immunization to their child's middle or high school on or before the first day of school in August.

Child’s shot record, parent photo ID and insurance card are required  by the parent to bring to the appointment.
